Job Responsibilities:
* Perform thorough initial physical therapy evaluations for each patient.
* Develop a treatment plan for each patient based on initial evaluation findings while collaborating with the other providers that are involved with each patients plan of care.
*Implement treatment plan on a consistent schedule and adjust this schedule and plan according to patient progress.
* Thorough EMR documentation.
* Evaluate mobility, strength and current level of patient function, while setting goals to improve in these areas that are scaled to patient abilities and realistic outcomes.
* Report patient progress to the patient, other providers involved in care, and supportive staff involved in ongoing treatments.
* Develop an appropriate home exercise plan for each patient.
* Maintain professional and technical knowledge through attending continuing educational workshops; reviewing professional publications, etc.
* Maintain a clean, safe, and orderly working environment to effectively perform all required patient care.
Qualifications:
* Ability to provide continuous, quality patient care on a full-time patient load safely and effectively.
* Strong experience of administering a wide range of passive and active therapeutic activities including but not limited to: strong manual therapy techniques, passive & active range of motion with mobilizations, PNF, NMR, therapeutic exercise and a variety of taping techniques.
* Ability to direct supportive staff to watch over and assist in patients' therapeutic exercises preformed in the clinic.
* Willingness and ability to delegate to supportive staff manual therapy techniques preformed on patients.
* Knowledge of responsible use of facility equipment, best practices for equipment use, and preventative maintenance procedures.
* Physical ability to help supportive staff with patient transfers.
* Strong experience with a wide range of patient age groups having multiple orthopedic, neurological, and other medical conditions.
* Willingness and ability to work well with other healthcare providers including Doctors of Chiropractic, Nurse Practitioners, and Massage Therapists that will be part of each patients treatment plan.
* Doctoral Degree in Physical Therapy from a physical therapy program approved by the American Physical Therapy Association.
* Current active in good standing Physical Therapy license for state of Illinois.

Salary: $50.00 - $58.00 per hour
