4. Access to Care and Scheduling 4.1 Standard Appointment Hours (Weekdays 8:30 AM – 4:30 PM) - Appointments are required for all visits during these hours. - Time slots are scheduled in 30-minute increments. - Patients should arrive 10 minutes early. 4.2 After-Hours Walk-In Care (Weekdays 5:00 PM – 7:45 PM & Weekends) - No appointment is required, but scheduled visits are encouraged to reduce wait time. - Services are provided on a first-come, first-served basis during these times. 4.3 Telehealth Visits - Telehealth is available for non-urgent matters, including: - Routine follow-ups - Prescription refills - Minor illnesses - In-person visits are required for: - Physical exams - Labs, procedures, or urgent care - Any condition deemed clinically inappropriate for telehealth
5. Member Discounts 5.1 Nutrient Infusion & Hydration Therapy (NIHT): Members receive discounts based on transaction totals: - $100–$149 = $10 off - $150–$199 = $15 off - $200–$249 = $20 off - $250+ = $25 off 5.2 Medical Weight Management: Members receive an automatic $20 discount on all services and products. 5.3 Exclusions: Occupational Health Services are excluded from membership discounts and are billed separately. Details may be expanded in Exhibit A.
6. Automatic Renewal All apt+ memberships automatically renew on a monthly or annual basis depending on the subscription selected, unless cancelled by the account holder. You may cancel at any time by contacting our office directly. - No early cancellation fees - Cancellations must be made before the next billing cycle to avoid charges
7. Cancellation and Refunds - Monthly subscriptions: No refunds for fees, services, or products already paid. - Annual subscriptions: Eligible for prorated refunds based on unused full months. - Refunds are processed within 15–30 business days. APT Urgent Care reserves the right to cancel any membership for non-compliance, abuse of services, or non-payment.
8. Patient-Provider Relationship and Health Expectations As an apt+ member, you are entering into a patient-provider relationship that is based on shared responsibility and mutual respect. As your care team, we commit to: - Providing timely, evidence-based medical care - Maintaining open and confidential communication - Offering appropriate referrals when needed - Respecting your values and preferences
